HealthSnap has secured $25 million in growth financing as the healthcare technology company moves to expand its AI-powered virtual care platform and strengthen its position in remote patient care.
The Miami-based company raised the capital through a senior secured growth financing facility led by Eastward Capital Partners. HealthSnap said it will use the financing to refinance existing debt, strengthen its balance sheet, accelerate AI innovation and support the expansion of its commercial operations.
The financing also comes as HealthSnap continues to scale its Advanced Primary Care Management platform, which aims to help healthcare organizations provide continuous care to patients beyond traditional office visits.
HealthSnap Targets Growing Demand for Virtual Care
HealthSnap provides technology that helps healthcare organizations monitor and manage patients remotely. Its platform supports programs including remote patient monitoring, chronic care management, principal care management and Advanced Primary Care Management.
The company’s platform collects patient health information through connected devices and other digital tools. Healthcare teams can then use this information to monitor patients between appointments and identify when additional care or intervention may be needed.
HealthSnap also provides tools for clinical workflows, patient engagement, population health analytics, and reimbursement management. According to the company, its platform integrates with more than 80 electronic health record systems. This approach allows providers to extend care beyond the traditional clinic setting while giving care teams access to information between patient visits.
New Financing Will Support AI Expansion
A major focus of HealthSnap’s new financing is the continued development of artificial intelligence across its platform. The company plans to expand AI capabilities across clinical decision support, workforce enablement, patient engagement and its Advanced Primary Care Management offering.
HealthSnap reported that its AI platform reviewed more than 3 million progress notes during 2025. The company says its AI systems work within clinician-defined protocols, while healthcare professionals retain control over clinical decision-making.
The company believes these tools can help care teams manage larger patient populations while reducing some of the administrative work involved in virtual care programs.
HealthSnap Serves More Than 80,000 Patient Programs
HealthSnap said it currently supports more than 80,000 active patient programs across 200 health systems and physician organizations. The company expects that number to exceed 100,000 active patient programs by the end of 2026.
HealthSnap’s announcement also highlighted relationships with healthcare organizations including Prisma Health, AdventHealth, Ascension Health, Sentara Health, Tampa General Hospital, UnityPoint Health, Baptist Health South Florida, Mount Sinai Medical Center and University Hospitals. These relationships form part of HealthSnap’s broader strategy to expand the use of virtual care technology across healthcare organizations.
2026 Financing Is Different From Its 2024 Series B
HealthSnap’s latest $25 million financing is separate from the company’s $25 million Series B announced in February 2024. The earlier Series B was an equity financing led by Sands Capital, with participation from Comcast Ventures and other investors. At the time, HealthSnap said the round brought its total funding to $48.5 million.
The 2026 transaction has a different structure. It is a senior secured growth financing facility led by Eastward Capital Partners rather than a new equity funding round. The financing provides HealthSnap with additional capital while also allowing the company to refinance existing debt.
CEO Highlights Next Stage of Growth
Samson Magid, Co-Founder and Chief Executive Officer of HealthSnap, said the new financing will help the company accelerate its AI strategy and expand its virtual care platform.
HealthSnap’s focus is increasingly centered on combining remote patient monitoring, clinical workflows and artificial intelligence to help healthcare organizations manage patients outside traditional care settings. The company plans to continue investing in its technology while expanding its commercial operations and Advanced Primary Care Management business.
HealthSnap Looks to Scale Virtual Care
The new financing gives HealthSnap additional resources as it enters its next stage of growth. With more than 80,000 active patient programs already supported, the company is targeting further expansion during 2026. Its planned investment in AI also reflects the broader shift toward technology that can help healthcare providers manage patient information and workflows more efficiently.
As HealthSnap expands its platform, the company will focus on growing its healthcare network, developing AI capabilities, and increasing the adoption of virtual care programs. The $25 million growth financing therefore gives HealthSnap both additional financial flexibility and capital to pursue its next phase of expansion in the growing virtual care market.
